Identity Theft - PASSPORT Program

The Worthington Division Police is pleased to offer assistance to victims of Identity Theft via the State of Ohio Attorney General's PASSPORT Program. This unique program provides victims of identity theft a method of demonstrating to law enforcement and creditors that their identity has been stolen, and to begin rehabilitating their credit history and identifying any fraudulent criminal charges.
Once verification of the victim is confirmed and a police report is taken, a specially trained Worthington police officer will aid the victim in completing and electronically filing a PASSPORT application. The officer will also provide the victim with identity theft materials provided by the Attorney General’s Office and can assist in providing additional identity theft information.
Victims of Identity Theft interested in obtaining a “PASSPORT” must have previously filed a police report with their local jurisdiction and must present a form of a governmental identification at the time of application.
This service is provided by “appointment only” and there are no costs or fees associated with the program. If interested, contact Officer Tammy Floyd at (614) 436-8634 or e-mail.
